submit the ICF first, so that we know you’ve started your onboarding. If you don’t submit the ICF, then
you may become lost in our system until you do so.
Your onboarding steps:
1) Fill Out Initial Contact Form
2) You’ll be directed to download the G8way Max blue app, sign the waiver, and take the assessment.
2) Once you receive your Wellness Report after taking the assessment, you will be prompted to click on a
confirmation button to download the second G8way Max app (white icon). Once you download both
apps, your onboarding is done.
